Announcement: APPLICATION FOR FELLOWS

1. Endorsement from the Chapter President. However , in the absence of the president or his failure to give endorsement, the PAMS governor can issue as such.
2. Attendance of 2 midyear plus 1 annual or 2 annual plus 1 midyear after diplomate conferment  ( Year of conferment is not included).
3. Copy of diplomate certificate properly signed by the diplomate for authentication. Those who have not received  their  diplomate certificate may present a certification from the Chapter President , However , in the absence of the president or his failure to give certification, the PAMS governor can issue as such.

1. Members of more than 2 years who failed to complete the minimum requirement as far as the attendance and dues are concerned, may  apply for conferment as fellow this coming Annual convention. However, your certification will be issued upon completion of such deficiency.
2. Members of more than 2 years who attended the Annual and Midyear convention by the break away group at Fort Bonifacio and Vigan may be allowed to apply for conferment as fellow provided they will submit their certificate of attendance and will sign the request for accreditation prepared by Atty. Ed Chico before the conferment.
